Parent/Guardians, please follow these six steps to correctly complete the camper medical form:

1. Fill out FAMILY INFORMATION and HEALTH HISTORY sections.
2. Sign and Date Parent's authorization Section and Tylenol Permission.
3. Take camper for medical examination no earlier than three months before camp starts.
4. Have licensed Physician fill out IMMUNIZATION HISTORY and MEDICAL EXAMINATION sections.
5. Have physician sign PHYSICIAN'S REPORT section.
6. Return form to the camp office no later than May 15th.

PLEASE NOTE:  New York State Health & Insurance Regulations do not permit any camper to attend any day camp unless a completed medical form is on file before camp is in session.

REMINDER: Please retain a copy for your records and proof of examination.
